AHCCCS (Arizona Medicaid & Medicare)
Aviva works with AHCCCS and serves members enrolled in Arizona’s Medicaid system. Services are typically covered under long-term care programs and require:
AHCCCS Complete Care (ACC) plans (when applicable to services)
Prior authorization from your AHCCCS health plan
Case manager or service coordinator approval
Eligibility through programs such as:
Arizona Long Term Care System (ALTCS)
Insurance Verification & Authorization
Aviva can assist with:
Understanding covered services
Verifying AHCCCS eligibility
Coordinating with your case manager or health plan
Obtaining prior authorizations

We accept services covered under Medicaid Waiver programs, including those designed to support long-term care, home-based services, and community-based living. Eligibility is determined through state programs and assessments.
Aviva accepts members enrolled in ALTCS, which provides long-term care services for individuals who are elderly or have disabilities and meet medical and financial eligibility. Services must be authorized through your assigned ALTCS case manager.
Managed Care Organizations (AHCCCS Plans)
We coordinate services with AHCCCS-contracted health plans, including (depending on eligibility and authorization):

- Arizona Complete Health
- Banner – University Family Care
- Molina Healthcare of Arizona
- Care1st Health Plan Arizona
- American Indian Health Plan (AIHP)

Medicaid Waiver Programs
Arizona Long Term Care System (ALTCS)
We accept services covered under Medicaid Waiver programs, including those designed to support long-term care, home-based services, and community-based living. Eligibility is determined through state programs and assessments.
Aviva accepts members enrolled in ALTCS, which provides long-term care services for individuals who are elderly or have disabilities and meet medical and financial eligibility. Services must be authorized through your assigned ALTCS case manager.
Tribal and Indian Health Services (IHS)
Aviva serves Native American communities and may coordinate with:
- Indian Health Service (IHS)
- Tribal health programs
- Tribal ALTCS case management
- Tribal Regional Behavioral Health Authority (TRBHA)
We respect tribal sovereignty and work within approved systems to ensure culturally appropriate care.
Veterans Programs
Eligible veterans may qualify for benefits such as:
- VA Aid and Attendance Pension
- Veterans-directed care programs
We can assist in coordinating services and providing required documentation.

Private Pay (Self-Pay)
Aviva offers private pay options for Home Care and Adult Day Care services. Families may choose to pay directly when services are not covered by insurance or AHCCCS. Payment arrangements may be available depending on service type.
